In a state with only three local health systems, accessing specialized hypertension care can be a significant challenge for many Delawarans.

The proposed funding will be aimed at forming peer support groups designed specifically for those navigating hypertension, targeting communities struggling with access to support. Peer support structures are particularly vital for populations who may feel isolated in their health challenges, as they foster an environment of shared experiences and mutual encouragement.

Organizations applying for funding must demonstrate their capacity to conduct outreach within these communities and facilitate the formation of effective support networks. Applicants should include local data that highlights the need for such support and outline their strategies for fostering engagement among participants.
